Spanish speaking hospice team
The all-new Spanish speaking hospice team is designed to provide care to
patients in their native language - Spanish . The entire team (nurse, social
worker, home health aide, chaplain and volunteer) speaks Spanish.
ADULT PROGRAM
HealthCare Dimensions Hospice services for adults are designed to provide medically appropriate care,
as well as social work and
chaplain services deltasone online. Medical services are coordinated with the attending physician synthroid. The following
services are available:
Pain Management and Symptom Control
Nurses Available 24 hours-a-day, 7-days-a week
Social Work Counseling & Assistance
Pastoral Counseling
Home Health Aides
Give personal care to the patient under the supervision of the Primary Nurse. Home Health Aides are assigned based on the personal care
needs identified in the patient's plan of care. They assist the patient with activities of daily
living, for example; bathing, getting dressed, walking and eating.
Trained Volunteers
Volunteers may assist with shopping, errands, babysitting, light housework.
Bereavement Support
for 13 months following the death of an adult patient.
PEDIATRIC PROGRAM: HOSPICE FOR KIDS
A unique program designed to meet the medical and psychosocial needs of terminally ill children and adolescents. Medical services are coordinated
with the attending physician. The focus is on the family as the unit of care
including siblings and other extended family. The following
services are available:
Pain Management and Symptom Control
Nurses Available 24 hours-a-day, 7-days-a week
Social Work Support for the Entire Family
Pastoral Counseling
Home Health Aides
Give personal care to the patient under the supervision of the Primary Nurse. Home Health Aides are assigned based on the personal care needs identified in the patient's plan of care. They assist the patient with activities of daily
living, for example; bathing, getting dressed, walking and eating.
Trained Volunteers
Volunteers may assist with shopping, errands, babysitting, light housework.
Bereavement Support
for 36 months including expressive therapists, sibling support and parent
groups.
