HealthCare Dimensions Hospice is a program of comprehensive medical, social and spiritual care for child and adult patients with terminal illness. When aggressive treatment is no longer appropriate, hospice care focuses on keeping the patient comfortable and free of pain. Physicians, hospitals, insurance companies and case managers  refer patients to hospice. Patients and/or family members may also self-refer.

HealthCare Dimensions Hospice provides family-centered care at home, in extended care facilities and in hospitals in Boston and surrounding communities. The hospice team is made up of the patient’s primary care physician, a registered nurse, a medical social worker, a home health aide, a pastoral counselor and a volunteer. Services are based on identified needs and the wishes of the patient.

Bereavement services, including grief counseling, are available to the family and close friends for 13 months following the death of an adult patient and 36 months after the death of a child.

Payment is based on a reimbursement system that covers all team services, medical equipment and supplies, drugs related to the primary hospice diagnosis and, if needed, coordination of acute inpatient hospital care. No patient is turned away because of an inability to pay for services.

HealthCare Dimensions Hospice is a not-for-profit, non-sectarian, Medicare and Medicaid certified, JCAHO (Joint Commission on Accreditation of Healthcare Organizations) accredited, licensed hospice provider.
