Hospice addresses the unique physical, social and spiritual dimensions of end-of-life care, offering palliative
(comfort-oriented)
care for symptom management and control of pain, while neither hastening nor postponing death.
Who is eligible for hospice services zerit?
Hospice care is appropriate for anyone in the final stages of a terminal illness.
Who provides care?
An interdisciplinary team consisting of the Primary Physician, a Medical Director, Registered Nurses, Social Workers, Home Health Aides, Chaplains,
Volunteers and Bereavement Staff.
Where is hospice care provided?
Care is provided in the patient's home, in an extended care facility, or, if necessary, in a hospital.
Who pays for hospice services?
Medicare, Medicaid and most private insurers cover hospice services.
What expenses are covered by hospice?
Coverage includes visits by the interdisciplinary team, medical equipment, supplies and medications determined necessary by hospice, and related to the
terminal diagnosis.
Who may make a referral?
Physicians, patients, family, or any member of the health care team may make a referral to hospice.

From the North - via Route 128 (I-95)
Take Rte 128/I-95 South to Exit #26. Stay to the Right and follow Route 20 East
to Waltham. At the 5th set of Lights (Fleet Bank on right, and Spoke N Wheel on
left), take a Right on Prospect Street. At the 4th set of Lights (after Nova
Biomedical and the bridge over the Charles River), take a Right on Crescent St.
Take the first Right on Woerd Avenue. After the first apartment building on the
left, take a left into our driveway and small parking area. Our Main Entrance is
set back at the end of the driveway. Parking is allowed in designated parking
spaces only. For additional parking, take the first left after the small parking
area near our main entrance.
From the South - via Route 128 (I-95)
Take Rte 128/I-95 North to Exit #24 (Route 30 Newton/Wayland). **
From the East - via Route 9
Take Route 9 West to Rte 128/I-95. Take ramp on the Right to Rte 128/I-95 North
to Exit #24 (Route 30 Newton/Wayland). **
From the East - via Mass Pike (I-90)
Take MA Pike West to Exit #15. Pay toll. Follow signs for Rte 128/I-95 North.
Stay in the right lane. Before getting on Rte 128, take Exit #24 (Route 30
Newton/Wayland). **
From the West - via Mass Pike (I-90)
Take MA Pike East to Exit #14. After toll booth, follow signs to Route 128/95
North. Stay in the Right Lane. Before getting on Rte 128, take Exit #24 (Route
30 Newton/Wayland). **
** At the end of the Exit #24 ramp, take a Right on Rte 30 East (Newton
Marriott will be on your left). At 4th set of Lights (Auburndale Liquors &
Dunkin Donuts on the right, Mobil Station on the left), take Left on Lexington
Street toward Waltham (Lexington St. turns into Moody St 0.1 miles).
At the 4th set of Lights (Burger King and Mobil Station on the left), take Left
on Crescent St. Take 2nd Left on Woerd Avenue. After the first apartment
building on the left, take a left into our driveway and small parking area. Our
Main Entrance is set back at the end of the driveway. Parking is allowed in

PARKING: You may parallel park along the right side of the driveway, or in front of our building. Also, as you drive down our
driveway, take the first left which brings you behind the stone church, and parking is available in the first
4 spaces on the
left and
the first 6 spaces on the right. Metered parking is available on the street.
