HealthCare Dimensions Hospice relies on the generosity
of others for the critical funds necessary to continue to
provide caring and compassionate end-of-life care. The frequency
and intensity of hospice care and services provided at this
difficult and challenging time for the patient and family
are, by their very nature, far more costly and often exceed
the amount of the insurance reimbursement. Yet, in keeping
with our philosophy, no one is denied care because of ability
to pay. This is why gifts and contributions are so very
critical to the provision of our services.
HealthCare Dimensions Hospice receives and accepts gifts
from individuals, families, social service organizations,
churches, service clubs, foundations and corporations, often
in memory of or in honor of a loved one or friend who was
cared for by HealthCare Dimensions Hospice terramycin. Individuals
also help ensure the future of hospice care by including
HealthCare Dimensions Hospice in their wills.. aricept. making gifts
of stock, personal property or real estate.
Hospice can mean many things to different people. But
what HealthCare Dimensions Hospice does is to help
patients and their families through an enormously
difficult time by providing compassionate care and
support that allows familes to say goodbye with dignity
and love.
Your generosity supports our nurses, home health aides,
chaplains, social workers and volunteers to continue to
bring essential physical, emotional and spiritual
support to hundreds of patients and their families every
year. Whether it is helping a patient control their
pain, listening to a patient’s concern about leaving
loved ones behind, or having a volunteer sit vigil so a
patient doesn’t have to die alone, know that your
support makes these wonderful programs possible.

memorial contributions be directed togoals. Gifts may be
directed to the Urban Hospice, generosity of friends and
supporters to sup-port our free and subsidized care
program.

CASH OR PERSONAL PROPERTY
Gifts of cash or personal property are the simplest ways
to support HealthCare Dimensions Hospice’s mission.
These gifts transfer cash, securities, personal
property, real estate, life insurance or other personal
assets immediately from you, the donor, to HCD Hospice.
These gifts are tax deductible to the extent allowed by
law.
BEQUESTS
Many donors also make gifts through their wills. A
bequest is often the most realistic way for donors to
provide a substantial contribution to HealthCare
Dimensions Hospice in the future. Many types of bequests
are available to help you specify how your assets are
distributed. Please consult with your estate planner or
legal advisor to determine a plan of giving that is most
meaningful to you. Also, please notify us if you plan to
include HCD Hospice in your will, so that we may include
you in our Legacy Society.
MEMORIAL GIFTS
A memorial gift in the name of a loved one can be a
lasting tribute for the future quality hospice care.
Families may also specify in funeral notices that
memorial contributions be directed to HealthCare
Dimensions Hospice in addition to, or in lieu of
flowers.
THE HONOR ROLL
Many patients and family members wish to show
appreciation to their nurses, chaplains, social workers,
home health aides and volunteers who have provided them
with compassionate, exceptional care. The HealthCare
Dimensions Hospice Honor Roll pays homage to staff
members who have been recognized by patients and their
families.
LIFE INSURANCE
A donation of a life insurance policy is an-other way to
give a substantial gift to HealthCare Dimensions
Hospice. To maximize your gift, you may contribute
premium expenses on the policy, naming HealthCare
Dimensions Hospice as owner and beneficiary, and receive
an additional tax deduction for the premiums.
EMPLOYEE MATCHING GIFTS
Many businesses support the charitable goals of their
employees by matching gifts. By participating in a
matching gifts program offered by your or your spouse’s
employer, you can dramatically increase the impact of
your contribution. Matching gift forms are available
from the employer.
DIRECTED GIFTS
Many patients and their families wish to direct gifts to
a particular program that has personal meaning or is
aligned with their charitable goals. Gifts may be
directed to the Urban Hospice, Hospice for Kids,
Bereavement, Chaplaincy or Free Care programs.
Directed gifts benefit specific programs through the
recruitment of staff, ongoing education and care for
patients who benefit from these specialized programs but
have no insurance and are unable to pay.
Donations in support of the HealthCare Dimensions Hospice,
as well as memorial donations, can be made to HealthCare
Dimensions Hospice either by check or credit card or by
using our on-line donation form.

Please include the following:
- Your check made payable to HealthCare Dimensions
Hospice
- If by credit card, include whether it's a
Visa or MasterCard, your account number, expiration
date and amount of the gift.
- If your donation is made in memory of someone, please
include the individual's name along with the names and
addresses of those you wish to receive acknowledgment
of your gift.
