Anyone can make a referral to HealthCare Dimensions Hospice -
physicians, case managers from HMOs and insurance companies, hospitals, as well
as a spouse, a sibling, mother/father, health care proxy, legal guardian or
other relative . Referrals can be made by phone at (781) 894-1100 or (800)
696-6424.
HealthCare Dimensions Hospice will be glad to make a free
informational visit to your home, nursing home or hospital to answer questions
and to determine if hospice care is appropriate cialis professional.
If the physician has not made the referral, HealthCare
Dimensions Hospice will contact him/her to discuss the appropriateness of

LIAISON PROGRAM
The Liaison Team at HealthCare Dimensions Hospice assists caregivers
and the health care team in evaluating patients for admission to hospice,
whether they live at home, in a nursing facility, assisted living facility or
hospital.
The Liaison Team works with patients, physicians, case managers, social workers,
family members or friends, who want to receive information about HCD Hospice or
would like to make a referral.
